Home and Community Care Program

The program provides health services based on client assessments conducted by a Nurse Case Manager, aiming to help community members remain independent. It values First Nation people as a key resource and offers services for all ages, including respite care, senior programs, nursing support, personal care, medical equipment loans, assistive device support, health education, medical transport, and nurse navigator services for children and youth (ages 0–17), focusing on assessments, screenings, and advocacy for Indigenous youth.

Services are provided at home after eligibility criteria are met.

Allocation of services is based on need and ensures that services are provided for as long as fully required.
