Regional Health Survey Availability Form
Attention Walpole Island Band Members residing on Walpole Island:
We are inviting you to participate in a Health Regional Survey at the Governance Building.
To take the survey, you will need to provide your health card number. The survey may take up to 50 minutes to complete.
Please follow these steps to participate:
Fill out an availability form to schedule your appointment (Weekdays: Monday-Friday 9:00am-3:00pm) .

As a token of appreciation, all survey participants will receive a $50 gift card upon completion of the survey.
